COVID-19 Vaccination Certificate (Vaccine Passport)

Vaccination certificates serve as proof of vaccination and will be issued to fully vaccinated persons. Two types of vaccination certificates are available:

  • International vaccination certificates: for use both within Japan and abroad. Please select this option if you plan on embarking on international travel.
  • Domestic vaccination certificates: for use within Japan only.

The Record of Vaccination of Covid-19 received at the time of vaccination can also be referred to as proof of vaccination for travel within Japan.
Persons with an Individual Number Card are eligible to apply for a vaccination certificate using a smartphone app. Please see here for more information.

Eligibility

Persons who satisfy all of the following:

  • Persons who are fully vaccinated and desire a proof of vaccination other than a Record of Vaccination (e.g., for international travel)
  • Persons who received the vaccination using a vaccination voucher issued by Saitama City

Cost

Free of charge

Application

The certificate issuance may take about a week after you submit the application and the certificate will be sent to you by mail.

Apply by mail:

Send all the required documentation to:

Apply in person:

Bring all the required documentation to the General Affairs Division in your respective ward office. Please be aware that certificates will not be issued on the same day of application submission.
Ward office hours: 8:30am – 5:15pm

Required Documentation

  1. Application form

  2. Passport (copy) *make sure the passport number and name are visible and readable.  *Driver’s licenses and other personal identification documents can be used when applying for a Domestic Vaccination Certificate.
  3. Vaccination voucher (copy) *or a document that shows your individual number (e.g. My Number card) if you have lost your vaccination voucher.
  4. COVID-19 Vaccination Record Card (接種済証) (copy) *see here
  5. A stamped return envelope with your name and address
  6. Identification including your name and address (copy)

Required Documents for Assigning an Agent

  1. A letter of attorney (ininjou) signed by the client or a document proving the proxy to be a qualified Statutory Agent (houteidairinin)
  2. Identification documents for the agent.

Please note:

  • If you lose your vaccination certificate, it can take up to two months for it to be replaced. Please try not to lose your vaccination certificate.
  • If you moved and received a vaccination in a city other that Saitama, apply for a vaccination certificate from the municipality that issued the coupon for that dose. (For example, if you received your 1st and 2nd vaccination coupons from Saitama City and City B, respectively, the vaccination certificate for your first dose would be issued by Saitama City while the vaccination certificate for your second would be issued by City B.)
  • Saitama City is unable to provide vaccination certificates for doses that were administered abroad. Please contact the embassy of the country in which you were vaccinated.

Convenience Store Pick-Up

You can access your vaccination certificate at a convenience store kiosk.

  • Printing fee (¥120/document)

Please note:

  • In order to get a vaccination certificate for international travel at a convenience store, you must have had at least one vaccination certificate issued either by mail (from a municipal office) or via the smartphone app sometime after July 21st, 2022. (You must renew your vaccination certificate after a change in passport number).
  • Except in cases of printing error, refunds are not available after issuance. If there is an issue with the quality of your vaccination certificate, contact the convenience store that issued it ASAP.
  • Check your document carefully before printing to ensure that all the information listed is correct. File for a correction here.
